Magnesium: Are You Taking the Right Kind?

Magnesium is having a moment, and for good reason — roughly 50% of Americans don't get enough from diet alone, according to data from the National Health and Nutrition Examination Survey (NHANES). But walking into a supplement aisle and choosing between magnesium glycinate, citrate, oxide, threonate, taurate, malate, and five other forms is genuinely confusing. The differences aren't marketing — they're chemistry. Each form delivers magnesium to different tissues at different rates, and choosing the wrong one means you're spending money on a supplement your body barely absorbs. Here's what the evidence actually says about each one.

Why magnesium deficiency is so common

Magnesium is the fourth most abundant mineral in the human body and a cofactor in over 300 enzymatic reactions, including energy production, protein synthesis, muscle and nerve function, blood sugar regulation, and blood pressure management. Dr. Andrea Rosanoff, director of research at the Center for Magnesium Education and Research, has spent two decades studying magnesium status in Western populations: "The recommended daily allowance is 310-320mg for women and 400-420mg for men. The average American intake is 250mg. That's not a small gap — it's a 20-40% shortfall."

The reasons are structural, not behavioral. Modern agriculture depletes soil magnesium — a 2004 study published in the Journal of the American College of Nutrition by Dr. Donald Davis at the University of Texas found that magnesium content in vegetables declined by 25-80% between 1950 and 1999 compared to USDA baseline data. Water treatment removes magnesium from drinking water. And the foods richest in magnesium — dark leafy greens, nuts, seeds, legumes — are the ones most Americans eat least.

Stress accelerates the problem. Dr. Leo Galland, an internist and author of The Allergy Solution, explains the feedback loop: "Stress hormones — cortisol and adrenaline — increase urinary excretion of magnesium. Low magnesium amplifies the stress response. You get stuck in a cycle where stress depletes magnesium and low magnesium makes you more sensitive to stress." This is why magnesium supplementation often has noticeable calming effects — it's not that magnesium is a sedative, it's that many people are deficient enough that correcting the deficiency restores normal nervous system function.

How to read a magnesium label

Before choosing a form, understand what the label means. A supplement that says "Magnesium Glycinate — 500mg" usually means 500mg of the magnesium glycinate compound, not 500mg of elemental magnesium. The elemental magnesium content — the amount your body actually uses — varies by form. Glycinate contains about 14% elemental magnesium by weight, meaning a 500mg glycinate capsule delivers about 70mg of actual magnesium. Citrate is about 16%. Oxide is about 60% — which is why it looks like a better deal per milligram, but its absorption rate is so low that you end up using less than glycinate.

Dr. Rosanoff recommends ignoring the compound weight and looking for the "elemental magnesium" line on the Supplement Facts panel: "That's the number that matters. Aim for 200-400mg of elemental magnesium per day from supplements if your diet isn't covering the RDA. More isn't better — excess magnesium is excreted by the kidneys in healthy people, but high doses can cause diarrhea, nausea, and in rare cases, hypermagnesemia."

For sleep and anxiety: Magnesium glycinate

Glycinate is magnesium bound to glycine, an amino acid that itself has calming properties. Glycine acts on NMDA receptors in the brain and has been independently studied as a sleep aid — a 2006 study in Sleep and Biological Rhythms by researchers at the Ajinomoto Company found that 3g of glycine before bed improved subjective sleep quality and reduced daytime sleepiness. In magnesium glycinate, you get both the magnesium and the glycine benefit.

Absorption is excellent — bioavailability studies show 25-30% absorption rates, among the highest of any magnesium form. GI side effects are minimal because the glycine chelation protects the magnesium from reacting with stomach acid and causing osmotic diarrhea. Dr. James DiNicolantonio, a cardiovascular research scientist at Saint Luke's Mid America Heart Institute and co-author of The Magnesium Miracle (with Dr. Carolyn Dean), considers glycinate the default recommendation: "If someone asks me which magnesium to take and they don't have a specific condition, the answer is glycinate. Best absorption, fewest side effects, and the glycine adds value."

Typical dose: 200-400mg elemental magnesium before bed. Start at 200mg for a week to assess tolerance, then increase if needed. Take with a small amount of food to further reduce any GI effects.

For constipation: Magnesium citrate

Citrate draws water into the intestines through osmosis, which is why it works for constipation but can cause loose stools if you take too much. This is an intentional mechanism, not a side effect — magnesium citrate is specifically used in medical settings as a bowel preparation agent at high doses. At supplemental doses (200-400mg elemental), it provides both magnesium supplementation and gentle bowel motility support.

Absorption is good — around 25%, comparable to glycinate. Dr. Shanti Eswaran, a gastroenterologist at Michigan Medicine and specialist in functional GI disorders, often recommends citrate for patients with IBS-C (irritable bowel syndrome with constipation): "It addresses two problems at once — magnesium deficiency and slow motility. The dose-response for the laxative effect is predictable, so patients can titrate to their comfort level." Start at 200mg and increase by 100mg every few days until bowel movements are comfortable. If stools become too loose, reduce the dose.

For cognitive function: Magnesium L-threonate

Threonate is the only form with robust evidence for crossing the blood-brain barrier and increasing brain magnesium levels specifically. A 2010 study by Dr. Guosong Liu at MIT, published in Neuron, demonstrated that magnesium threonate increased cerebrospinal fluid magnesium levels by 15% and improved learning and memory in animal models — effects that were not seen with other magnesium forms at equivalent doses. The researchers proposed that threonate's unique molecular structure allows it to be transported across the blood-brain barrier via specific amino acid transporters that other magnesium forms can't access.

Human data is emerging. A 2016 randomized, double-blind, placebo-controlled trial published in the Journal of Alzheimer's Disease (n=44, ages 50-70) found that magnesium threonate improved executive function and working memory over 12 weeks. The participants were cognitively normal adults with subjective memory complaints — not dementia patients — and the improvements were modest but statistically significant.

The downside: threonate is significantly more expensive — typically $30-45 per month compared to $10-15 for glycinate. The elemental magnesium content per capsule is also lower (about 7%), meaning you need more capsules to reach the same elemental dose. For most people, glycinate provides adequate magnesium that incidentally supports brain function. Threonate is worth the premium if cognitive performance is your primary goal and you're willing to pay for the specificity.

For muscle cramps and exercise recovery: Magnesium malate

Malate is magnesium bound to malic acid, a compound naturally found in apples and involved in the Krebs cycle (the body's primary energy production pathway). Dr. Guy Abraham, a former professor of obstetrics and gynecology at UCLA, studied magnesium malate specifically in fibromyalgia patients and found that it reduced muscle pain and tenderness over 8 weeks at doses of 300-600mg elemental magnesium daily.

The theoretical advantage for exercise is that malic acid supports ATP production, so malate delivers both magnesium (which prevents cramps by regulating calcium channel function in muscle cells) and an energy substrate. Evidence is stronger for malate than for citrate or glycinate in the specific context of muscle pain and exercise-related cramping, though the difference is modest.

For heart health: Magnesium taurate

Taurate is magnesium bound to taurine, an amino acid with documented cardiovascular benefits. Taurine independently supports heart rhythm stability, blood pressure regulation, and endothelial function. A 2018 meta-analysis published in Hypertension Research by Dr. Xian Liu at Qingdao University found that magnesium supplementation reduced systolic blood pressure by 2-4 mmHg — a modest but clinically meaningful effect, particularly in people with mild hypertension.

The taurate form specifically has been studied by Dr. Mark McCarty, a nutritional researcher, who proposed that the combination of magnesium and taurine provides synergistic cardiovascular protection through complementary mechanisms — magnesium relaxes vascular smooth muscle while taurine stabilizes cardiac rhythm. Clinical evidence for taurate specifically (versus other magnesium forms) is limited, but the theoretical basis is sound and the taurine adds no downside.

Skip: Magnesium oxide

Oxide is cheap and everywhere — it's the form in most multivitamins, many budget standalone supplements, and nearly every magnesium product at the grocery store pharmacy. It's also poorly absorbed. A 2001 study by Dr. Gerry Schechter at the University of North Carolina, published in the Journal of the American College of Nutrition, found bioavailability around 4% — compared to 25-30% for glycinate and citrate. A 500mg oxide tablet delivers about 300mg of elemental magnesium, but your body absorbs only about 12mg. You'd get more magnesium from a cup of spinach.

The only legitimate use for magnesium oxide is as a short-term laxative at high doses, where the unabsorbed magnesium creates an osmotic effect in the intestine. For supplementation, it's objectively inferior to every chelated form. If your current supplement says oxide on the label, switch. The price difference is typically $5-10 per month — the cheapest health upgrade you can make.

How to choose

The form matters because magnesium itself is never supplemented alone — it must be bound to a carrier molecule, and that molecule determines where the magnesium goes, how well it's absorbed, and what secondary benefits (or side effects) you get. Don't overthink it:

Most people: Magnesium glycinate, 200-400mg elemental, before bed.

Constipation or IBS-C: Magnesium citrate, titrate from 200mg upward until bowel movements normalize.

Cognitive performance as primary goal: Magnesium L-threonate, 1,500-2,000mg compound (about 140-200mg elemental), split into morning and evening doses.

Muscle cramps or exercise recovery: Magnesium malate, 300-400mg elemental.

Cardiovascular support: Magnesium taurate, 200-400mg elemental.

Never: Magnesium oxide, unless you're specifically using it as a one-time laxative.

You can combine forms — glycinate at night for sleep and malate in the morning for exercise — as long as total elemental magnesium stays below 400-500mg daily from supplements. More than that doesn't help and increases the risk of GI side effects. And the best magnesium supplement in the world doesn't replace dietary magnesium: a cup of pumpkin seeds (168mg), a cup of cooked spinach (157mg), or an ounce of dark chocolate (65mg) all deliver magnesium in a food matrix that enhances absorption. Supplements fill the gap. Food builds the foundation.
